Filing a notice

• your name, address, telephone number, and email address;
• a statement that you have a good-faith belief the use is not authorized by the owner, its agent, or the law;
• a statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act on the owner’s behalf;
• your physical or electronic signature.

Counter-notice & repeat infringers

If material you posted was removed, you may submit a counter-notice with the same contact details and a good-faith statement. We terminate access for repeat infringers in appropriate circumstances.
